NOTIFICATION OF INFORMATION COLLECTION & PRIVACY PROTECTION

Your information is collected by the Therapeutics Initiative under the authority of section 26(c) of the Freedom of Information and Protection of Privacy Act (FIPPA) for the purpose of correctly linking you to your electronic Portraits and communicating with you regarding the Portrait program. The information you provide will be used to authenticate your identity and confirm the data linkages necessary to provide your digital prescribing Portraits. The Therapeutics Initiative may also use your information for the purpose of evaluating the Portrait program; your information will be anonymized before it is used for evaluation purposes.

The information held in the Portrait online system will be used as follows:

  • Email address will be used only for notifications of new portraits, Therapeutics Letters, and communications necessary to administer this site. You may update your communication preferences at any time.
  • Email address may also be used for account validation.
  • Name, College Licence number and MSP billing number will be used only for authentication - to ensure that the person getting the Portrait is in fact the person whose data is represented.
  • Neither your name nor other identifying information will be published in any form.
  • Your site usage (anonymized) may be used in the evaluation of the Portrait program.
  • MSP billing number is stored for the purpose of ensuring that your Portrait is correctly matched to you.
  • College Licence number is used for authentication and only stored in a de-identified version.


At any time, you may opt out of the Portrait program, at which point you will no longer be notified of new Portraits and no new Portraits will be created for you.
